Performance Improvement Interventions

The UF CQM performance improvement intervention program is designed for health plan members that are predicted to affect Medicare Advantage and Prescription Drug plans (MAPD and PDP) Star Ratings pertaining to the CMS Patient Safety Measures, HEDIS Measures, as well as other quality measures. This program is intended to complement health plans’ quality initiatives already in place and provide member-specific, clinical interventions with both patients and providers

  • Targeted interventions for members who have been predicted to impact adherence measure ratings
  • Telephonic assessment (both initial and follow-up) with members to identify barriers to adherence
  • Delivery of meaningful, patient-specific interventions designed to overcome barriers and improve adherence
  • Develop a customized health-plan specific toolkit of interventions that plan wishes to offer members

Client-Centered Solutions

  • Prescriber and patient outreach initiatives designed to target a specific plan or quality measure over a set period of time or on an on-going basis
  • Intermediary services providing a connection between patients, pharmacists, providers, and the plan with the goal of optimizing medication therapy
